Not sure if this is a bug or not.

What causes this error when trying to navigate to a page?

No tab in the instance. Instance 50505:127.0.0.1

I'm hitting errors like this a lot.

No page tab in the instance. Instance 60001:127.0.0.1

What causes it and is there any way to avoid this?

How often does it happen?

It happens in about 1 in 3 instances. Running 10 threads results in 3 of these. Even sometimes when running just 1 thread a "no page tab" instance can occur.

Unfortunately these "no page tab" instances cause all sorts of trouble with templates because the template is executed as if there is a page loaded with no data at all.

Actually it doesn't always happen at the time a new thread and instance starts. It seems even a currently working instance can go bad so to speak.
