Erro: run table object reference not set to an instance of an object

i run my project for very long time. it worked well in the most of time. but it has a problem sometimes.
The error log said: run table object reference not set to an instance of an object (attached)
when i open the table(.xls), it said that "The file format and file extension specified format is not the same, the file is broken"

what is the problem?

до you access to file manually when ZP try to add data to it?> is it open?
is at changed or read by another applications at the same time?
how do you add data there? what actions do you use? do you use "save to file" actions somewhere?
