Any thoughts how to reproduce?

Really no any thoughts about this. Because looks like it is not global bug. Because this pic is prom my server, but i can not reproduce this bug on my home PC. Both systems using same OS.
Looks like this is something related to this old bug when toolbar disappeared at all. As i remember this too was there on just of some PC's and sometime.
